SmartPractice® CanadaTM is proud to be the exclusive North American distributor of the allergEAZE patch test system, which includes allergens, patch test chambers and several ancillary products.
Please note that allergEAZE products require supervision of a currently-licensed physician in good standing, and orders should be shipped to business-affiliated addresses

A SmartPractice® Canada Product Consultant will contact you by phone or email to confirm receipt of your request, and discuss product availability, pricing, international delivery and method of payment.
Customers outside the U.S. can also request a quote or place an order directly with a SmartPractice® Canada Product Consultant, available Monday through Friday, 9am –7pm EST at 1.866.903.2671.
Due to the complexity of international shipping, product price quotes for customers outside the U.S. will be based on the item(s) selected, and shipping/delivery charges. Price quotes will be in U.S. dollars, and valid for 30 days, unless otherwise noted.
For all customers outside the U.S., SmartPractice® Canada currently accepts MasterCard and Visa. Other payment options are available to Canadian allergEAZE customers; please ask your SmartPractice® Canada Product Consultant for additional information.
SmartPractice® Canada reserves the right to determine pricing and payment methods, and to modify these policies without notice.
Shipping
SmartPractice® Canada will attempt, as much as possible, to anticipate potential shipment and delivery problems, but will not be held responsible for delays due to severe weather, unforeseen circumstances, or customer-based delivery problems. For customers outside the U.S., product delivery time will be estimated based on product availability, shipping options and destination country.
allergEAZE Allergens
are shipped directly to customers outside the U.S. However, some products may not be deliverable to all international destinations, based on country-specific restrictions. Expedited shipping may also be available in some countries (additional charges may apply) – please ask your Product Consultant for more information.
Customers are responsible for proper storage and handling of allergens after delivery. Refrigerated storage (2o * 8o C, or 35.6o * 46.4o F) is

are shipped via Fed Ex ground to Canada. For other destinations outside the U.S., or expedited delivery (additional charges may apply), please ask your SmartPractice® Canada Product Consultant for additional information.

Returns
In general, allergEAZE products are not returnable. However, SmartPractice® Canada will attempt as much as possible to resolve problems to the customer’s satisfaction. If products arrive damaged, or appears defective, please notify SmartPractice® Canada immediately at 1-866-903-2671.
